Adult Ballet Class Reservation

Be sure you have already completed the Adult Ballet Class Registration Form. The registration form only has to be completed ONCE. After the first time you complete that form, you can just skip ahead to this reservation form moving forward.

When filling out the form below, make sure you are entering the same name and email address that we have on file for you.

Single class - $35 plus tax

5-Class Pack - $150 plus tax (only $30 per class!)

10-Class Pack - $250 plus tax (only $25 per class!)

I already paid for a package and still have classes left. I am only reserving my class spot below.

Please note: If you purchased classes and are not able to use them all before the end of the season, you can use them towards any adult dance classes next season. We will NOT be offering refunds for unused classes.

Reserve your Classes

Please only reserve your spot in a class that you know you are able to make. We understand that things pop up that might cause you to miss a class, however we ask that if you are unsure if you will be available, please don't take a spot in the class.

CANCELLATION POLICY: Please email office@maddiesdancestudio.com at least 24 hours in advance to cancel your class reservation. Any cancellations after 24 hours or no-shows will incur a $10 (plus tax) late fee to the card on file. If you no-show, we will also remove a class from your paid class sessions.

Note: Classes will still happen with only one reservation.
