Please fill out one form per dancer. If there are multiple dancer's in your family, please ensure the primary parent information is the same so you are only charged for ONE registration fee per family.

TUITION INFORMATION

Season 2 begins on Monday, September 8th and goes through Saturday, June 6th. We will follow the Greenwich Public Schools break calendar. Our spring recital will take place on Sunday, June 7th.

Caterpillars/Butterflies Class - $899 (plus tax)

1-Hour Class - $999 (plus tax)

PICK YOUR DANCER'S CLASSES

WAITLIST: If the class you are trying to register for is full, you will be added to the waitlist. You will not be charged unless a spot opens up. You will be contacted to confirm whether or not you would still like to register for the class should a spot open. You will have 24 hours to decide if you would like to take the spot before it is offered to the next person on the waitlist.

AGE GROUPS: The dancer MUST be the first age of the class by the first day of classes to join the class. For exceptions, please contact us first for approval before registering. We WILL NOT put registrations through if dancer is in the wrong age group without prior approval.

PAYMENT INFORMATION

Once the form is submitted, please allow for some time for the team to review your registration. Payment will be charged after review of this registration form. Only $50 registration fee will be charged (unless you choose option to pay in full today).

Tuition is DUE IN FULL by August 1st.

Once this form is submitted, it must be reviewed by us. You will receive an email confirmation and receipt once we receive and put your registration through our system. Please note, this may take a few days.

STUDIO POLICIES

Refund Policy: A full refund (minus the registration fee) will be provided if you choose to withdraw within the first 3 weeks of classes. After the 3-week grace period, refunds will only be considered for medical reasons (with a doctor’s note) or if the family relocates more than 15 miles away from the studio (with proof of new address).

Registration Fee: A non-refundable registration fee of $50 per family is required to enroll in classes.

Tuition Payment and Spot Reservation: Tuition will be charged to your account, and payment must be made by the specified due date. If we are unable to process your payment, there is a risk of losing your spot in the class. In the event of a waitlist, your spot will be forfeited and offered to the next person on the list.

Class Cancellations: In cases where classes are canceled (e.g., due to weather, instructor illness, etc.), we have built in three "cancellation days" for each class. Should more than three classes be canceled, makeup sessions will be scheduled in June. For example, if five classes are canceled, two makeup classes will be scheduled in June.

Missed Classes: Makeup classes are not offered for absences due to personal reasons.

Class Observations: In order to create a focused and supportive environment, parents and guardians are not permitted to sit in the dance studio and observe classes during their dancer's session.

Dress Code Compliance: All dancers are required to adhere to the designated dress code for their respective class.

​Class Cancellations Due to Low Enrollment: Maddie's Dance Studio reserves the right to cancel any classes due to low enrollment. In such cases, families will receive a full refund, including tuition and registration fees.

WAIVER & RELEASE OF LIABILITY

In consideration of the risk of injury associated with participating in dance (the "Activity") and my desire to participate, I voluntarily enter into this Waiver and Release of Liability. I waive any rights, claims, or causes of action arising from my participation in the Activity.

I release Maddie's Dance Studio LLC and Sportsplex Stamford LLC, located at 49 Brown House Rd, Stamford, CT 06902, along with their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, and assigns (collectively, "Releasees"), from any liability for physical or psychological injury resulting from my participation.

I understand and accept the risks of the Activity, which may include physical or psychological injury, pain, suffering, illness, disfigurement, disability (including paralysis), economic or emotional loss, and death. These risks may arise from my own or others' negligence, travel conditions, or conditions at the Activity location. I assume all related risks, known and unknown.

I agree to indemnify and hold harmless the Releasees against any claims or actions, including legal fees, related to my participation. I acknowledge that Releasees are not responsible for any errors or omissions by those conducting the Activity. I authorize Maddie's Dance Studio LLC and Sportsplex Stamford LLC to provide emergency medical care if needed and agree to cover all associated costs. I understand I should have my own health insurance.

I acknowledge that the Activity may involve physical and mental challenges with potential for serious injury or death. I will participate only if I am medically able and properly trained and will follow the decision of the Activity officials regarding my participation.

I have read and understand this Waiver and Release of Liability and agree to release Maddie's Dance Studio LLC and Sportsplex Stamford LLC and their affiliates from all claims, including those arising from ordinary negligence. This Release is governed by Connecticut law and supersedes any prior agreements. I agree to be liable for any damage to equipment or facilities resulting from willful actions or negligence.

This Waiver and Release remains in effect for the duration of my participation. The agreement is made voluntarily and will be interpreted as written. If any part is deemed invalid, the remaining provisions will remain in effect, and invalid provisions will be enforced as limited.

I affirm that I am 18 years or older and am signing this agreement freely, having read and understood its content.
